Fees & Clinic Policies
Appointments are available Monday, Thursday and Friday from 10am – 5pm (by appointment) in the Kingston office on Pierce Street. To schedule, please call 570.287.9955.
Initial visits are up to 90 minutes and consist of review of health history, health concerns and goals.
Return visits are up to 45 minutes and consist of following up on the previous visit and any new concerns. It also is a time to alter the treatments if needed.
Any laboratory fees or natural pharmacy items that may be recommended are not part of the appointment fee.
Phone Consultations
Dr. DeLuca is highly experienced in addressing a wide variety of conditions. She understands there might be questions, so she offers a free 5-10 minute phone consultation to learn more about her practice, natural therapies and if this approach is right for you. No recommendations will be made. Please all 570.287.9955 to schedule.
Phone consultations to clarify the current recommendations of 5 minutes or less are not charged.
Phone consultations are normally scheduled like regular appointments, but we welcome you to call anytime in the event of questions or concerns. We are dedicated to providing you with the highest quality care, which often requires us to spend time charting and developing plans beyond the length of the phone call. Due to time constraints, scheduled and unscheduled phone consultations that exceed 5 minutes are charged per minute at the regular hourly rate.
What you need to bring with you to your appointment
If you have had any labs or imaging done within the past year, it is helpful to have a copy of them with you. Also, be sure to write down any questions or concerns you may have ahead of time so that you do not forget to ask during your visit. Lastly, bring with you a list of all prescriptions and supplements that you take on a regular basis, including brand name and dosage. Or, simply put them in a grocery bag and bring them along with you.
For your convenience, I have provided printable forms to be filled out prior to your appointment. They may be mailed or emailed ahead of time. You will find these forms at the “clinic forms” link along the top of the page.
On Time policy
Appointment length will not be extended due to late arrival. I am happy to mail or email patient forms to you if you are unable to download them from our website. You may fill forms out at your convenience before your first appointment. This enables me to maximize my time with you.
Financial policy
A credit card is required to hold appointment time. No eceptions will be made. We do not charge your card until the visit and you can choose to pay with cash, check or credit card or prepaid gift certificate.
Payments are expected upon receipt of professional services and natural pharmacy items. Payments can be made by cash, check or credit card (Visa, Master Card or Discover)
Cancellation policy
A 24-hour notice is required for cancellation, without a charge. Failure to give a 24-hour notification of cancellation, of your appointment, incurs a charge of the scheduled appointment time. If you are new the clinic, you will be charged the rate for 90 minutes. If you are established, you will be charged the rate for 45 minutes.
This also applies to No-Shows (not appearing for your appointment), if you do not show up for your scheduled appointment time, you will be charged cost of the visit.
Personal emergencies will be given reasonable courtesy consideration. This is at the discretion of the doctor.
